Watermark at Home is a national home health and private duty services company. With its parent company, Watermark Retirement Communities, it has been providing unique senior living solutions for more than 20 years.
What is home health care?
Home health care orchestrates comprehensive medical assessments and treatments, including effective symptom control, to reach immediate and short term clinical goals. It also provides education and training that allows clients to integrate new self-management skills into their daily lifestyle. Both are important in the recovery of optimal wellness and vitality.

Home health care begins with an initial consultation between one of our Registered Nurses and your physician. Together they will develop the best care plan for you.

After a care plan has been developed, Home health care specialists come into your home to provide health care services and/or personal assistance.
What services are offered?
Home health care is provided by a Watermark At Home team that includes Registered Nurses (RN), Licensed Practical Nurses (LPNs), Certified Nursing Assistants (CNAs), certified Occupational, Physical and Speech Therapists, as well as Certified Social Workers (MSWs).

Each member of the team heads up a different element of your care.

This team will provide a range of services, including medication administration, wound care, pain control, dressing, bathing, toileting, feeding, as well as a full range of personal care and companionship services.

All care is provided under the supervision of a Registered Nurse who will make an ongoing, written nursing assessment.
How to pay for home care
Home health care can be paid for in a number of ways, and a combination of payment means is often used.

Because Watermark at Home is Medicare-certified, Medicare will pay for most care delivered with the primary goal of improving a current health condition so that you can live independently again. Specific conditions must be met, and your care supervisor will go over those with you.

For that portion of care that does not meet the Medicare requirement, or if you do not qualify for Medicare, then expenses may be paid by private long-term care insurance or private funds. This is most often the case if services rendered are not medical.

A member of your team will assist you in navigating these financial matters.

We'll work with you and your doctor to evaluate your health care needs and write your plan of care that will tell you what home care services you need.

If you or your family member will be leaving a hospital, skilled nursing facility or rehabilitation center, Watermark at Home can coordinate with the discharge planner to set up a home health care plan.
